Microbrachis pelikani is a Leptospondyl amphibian from Carboniferous Czech Republic. It had a long spine and short limbs of a water-dwelling amphibian, and like some modern amphibians (such as axolotl) it didn't undergo full metamorphosis, but retain larva-like characteristics (such as the external gills) throughout its life.
